The Clinic collects the basic contact information of clients and customers, including their full name, address, email address, contact number, birthday together with the services that they would like to avail. Contact information collected shall be used by the Clinic for documentation purposes and for proper labeling and releasing of results. All information will be kept confidential in accordance with the Republic Act No. 10173, also known as the Data Privacy Act of 2012 (DPA).
